Nominations must be received by October 1, 2010

2010 Nomination Forms are Provided in Two Ways:

2. Click here to download a form in rtf format that can be filled out with your PC. The completed form can either be mailed to Thurmont Lions Club, Attn: VOTY, PO Box 306, Thurmont, MD 21788, or attached to your e-mail that should be sent to either slong95@comcast.net or skfavorite@comcast.net. Please use the words "VOTY Form" as the subject heading.

Paper copies of the nomination form are also available at the Thurmont Regional Library, 76 East Moser Road, Thurmont, MD 21788
